Patients/guarantors with annual household incomes of 100% or less of the Federal Poverty Guidelines are eligible for uncompensated care. Patients/guarantors with annual household incomes above 100%, but less than 250% of the Federal Poverty Guidelines may be eligible for discounted fees as outlined below. If you would like to apply for financial assistance, please complete the application found below, including proof of household size and income, and return it to our business office at 312. S. Adams Ave., Terry, Montana 59349 or mail it to our business office at P.O. Box 156, Terry, Montana 59349.
